The Franchise Application Form does not need to be filled out with the NDA or the WRTS Inquiry Questionnaire Form, but it is necessary in order to become an owner. This form is used to prequalify you as a gym owner based upon all of your financial information, business experience, commitment to the gym, and responses from references.
